Preparation for your appointment

Expect Dr. Sokolowski to spend time getting to know you and your history at your first visit.  She will review the history form you will have completed prior to the visit and help you to formulate a plan.  Please download and complete new patient forms below (please make sure to download all 11 pages) prior to your appointment.  Please bring photo ID and your insurance card(s) with you to your appointment, and plan on arriving a few minutes early to finish the new patient check-in process.  Please call if you have any questions.  (610) 391-9180.
